Pay Range: $43.96 - $43.96 per hour

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Social Worker Licensed (LSW) provides direct clinical services in our behavioral health adult outpatient program, such as initial intake assessments, and individual and group counseling services to adults with Serious Mental Illness SMI, including those with a co-occurring substance-use diagnosis in our Adult Outpatient department.

Qualifications:

Required:

  • Master's Degree in Social Work

Preferred:

  • LCADC 
  • One year clinical experience in a substance abuse setting

Certifications and Licenses Required:

  • NJ Licensed Social Worker (LSW) 

Scheduling Requirements:

  • Per Diem (PD)

Essential Functions:

  • Interprets nature of illness and prescribed course of treatment to patients and families
  • Communicates family issues to other staff members
  • Participates in planning conferences
  • Identifies substance abuse/use issues and incorporates into the TX plan identifying triggers, support mechanisms and 12-step participation if appropriate
  • Participates in supervision activities, as directed
  • Maintains up-to-date and complete records of patient information according to established standards
  • Documents progress notes within 24 hours of DOS
  • Understands the payer identification process and ensures that services delivered are accurately authorized
  • As a member of the multidisciplinary team, assists in evening and updating treatment plan
  • Involves families/referral sources in all treatment endeavors unless contraindicated
  • Adheres to department's practice guidelines
  • Maintains current knowledge of professional practice standards
  • Maintains current knowledge of relevant addictions tx and research
  • Performs random UDS on patients
  • In collaboration with treatment team, implements the discharge plan
  • Collaborates with other Hospital programs and community agencies to promote coordination and continuity of patient and family care
  • Performs the psychosocial assessment, which includes family dynamics and treatment needs according to specified outpatient/inpatient standards
  • Designs and implements treatment plans, including individual, group and/or family psychotherapy and evaluates and documents patient's outcomes according to departmental policy and procedures
  • Conducts individual, family, and group treatment according to established standards/practice guidelines
  • Recognizes the need for and implements a strength-based framework in treatment planning
  • Collects and utilizes outcome data to support treatment planning/modifications
  • Reviews assessment data including identified medical needs and makes appropriate and timely referral
